The toll allows underutilized homecare nurses and support workers to complete mandatory on-site homecare forms via tablet. The company is based in Toronto, Canada. We match homecare-seekers with homecare-providers based on a comprehensive self-assessment that can be completed at home or at a hospital bedside. Thorough completion of the self-assessment is incentivized every step of the way with free or reduced-rate products and services that are directly related to the homecare-seeker’s illnesses or chronic conditions. Homecare providers are delivered 'hot leads' direct to a custom-built, homecare-specific, customer relationship management tool (CRM).
